Consumers can get organized and health care professionals can improve efficiency with the help of a new Web site offering free printable
medical forms. FreePrintableMedicalForms.com has launched with more than 50 health-related forms, trackers, logs and charts, all ready to download and print.
There are forms for individuals, family caregivers, health care professionals, and for use in school settings. Just a few of the forms are: a school physical report form, T.B. test results form, patient sign-in sheet, headache symptoms tracker and medication log.
"My hope is that consumers can use these forms to get organized as well as stay on top of their health care needs by keeping track of things such as blood sugar levels, allergy symptoms, quality of sleep and other issues," said Kevin Savetz, the site's creator. "And, if you or someone you care for is living with an illness or disease, either at home or in a hospice or assisted living setting, easy-to-read forms can help communicate crucial information to medical professionals."
FreePrintableMedicalForms.com also has forms intended to be used in a medical practice or other professional environment. A
blood pressure chart, pain level chart and immunization chart are just a few of them.
There are also everyday forms for keeping track of weight loss and
exercise tracker, plus food journals. There are printables related to family planning and pregnancy, such as a
fertility chart. And there are trackers to assist nursing mothers as well as to track babies' growth and development.
There is an entire category of forms for people with diabetes, such as
blood glucose charts.
All of the free PDFs can be instantly downloaded and printed and then written upon with pen or pencil, or typed onto using a typewriter. Another convenient option, for those who wish to be able to edit a form on your computer and use it multiple times, is a premium, $3.99 version of each form. The premium version, also instantly downloadable, is a .DOC users can save and customize in Microsoft Word. There is also the option to purchase the entire collection of editable forms in one download for $27.
"I created this site with virtually everyone in mind," Savetz said. "I fully expect to add more forms to the site as I hear from health care consumers and professionals regarding what printables they would find useful."
There are nearly 50 sites in the FreePrintable.net family of
free printables sites created by
Savetz Publishing, Inc., a company devoted to creating useful and informative web sites of interest to consumers and small businesses.
