An insurance expense tracker, virus symptoms log and a blank medical report are among the 20 new free printable medical forms, charts, journals and logs available for instant download on the Web site FreePrintableMedicalForms.com.

The site offers 75 free health care-related forms in all, from exercise and weight loss charts to medical office forms.

"FreePrintableMedicalForms.com has dozens of forms and charts for health care professionals and consumers," said Kevin Savetz, the site's creator. "There are logs intended to be used by caregivers as well as forms that will help patients take charge of their physical well-being."

Parents can make use of a handy fever diary that makes it easy to jot down a child's temperature as well as medications given (and dosages) as an illness runs its course.

There is also a PMS symptom tracker with room to record symptoms day-by-day.

Other new forms include a babysitter information form, chemotherapy side effects journal and sample drug and food labels.

All of the forms and charts are free to download and print individually as PDF documents (easily viewable by Adobe's free Acrobat Reader). Fully customizable .DOC versions (which can be opened and edited in Microsoft Word) can be purchased for $3.99. Another convenient option is to purchase the entire collection of both PDF and .DOC files as one download for $27.
